DUI Evaluation: After receiving a DUI (Driving Under the Influence) charge, a person will be required by the court and Department of Transportation to complete a chemical dependency evaluation. Upon completing this evaluation, the court is notified of participation and the DOT is given paperwork stating the evaluation is completed. Court requirements are then fulfilled. However, upon completion of the DOT paperwork, an individual has 6 months to fulfill any recommendation made during that evaluation to reinstate their license. Fee is $200.
Chemical Dependency Evaluation: This service is the same process as a DUI evaluation without any required paperwork to the Department of Transportation. It is the initial information gathering session in which the client and counselor meet to discuss family and personal history of drug/alcohol use, as well as other background information. People in need of this service may be self or externally referred. Fee is $200.
Evaluation at Outside Agency: Evaluations may be completed at jails, hospitals, or other requested agencies as appropriate at an additional fee due to transportation and added coordination. Fee is $300.
Substance Abuse Professional Evaluation: This is an evaluation requiring the appropriate documentation in the Return to Duty process governed by Federal Regulations and Safety Sensitive guidelines of the DOT. The evaluation is as comprehensive as a typical chemical dependency evaluation but will also include a referral to an outside agency to complete a required education or treatment program as determined by the evaluation. Following completion of the outside education/treatment component, clients are required to schedule a follow up session to complete the Return to Duty process. Evaluation and follow up session fees are $350 total.
16 Hour DUI Seminar: This service is an educational group utilizing the Prime for Life program. The information focuses on assisting clients in evaluating drinking on one's own terms and identifying safe versus high risk choices. The seminar is held once monthly over a period of 4 days. A person would need Report of Addiction Evaluation form completed from their most recent DUI evaluation at the time of admission. Fee is $250.
Individual Sessions: The client and counselor meet one-on-one and discuss what the client would like to gain from therapy in terms of drug or alcohol use. Education on Choice Theory and substance use is provided by the counselor. This process focuses on assisting the client in applying new information to daily choices and evaluating the effectiveness and fulfillment of those choices. Regardless of length of session, fee is $80.
Aftercare Programming may be completed in this format upon Choice Recovery Counseling receiving the full initial chemical dependency evaluation and discharge summary from the client's primary treatment.
Relapse Prevention Group: Group meets 530pm - 7pm each week. This is a weekly process group, focusing on day to day stressors and successes of recovery. This group is for people who have experience in recovery, as well as periods of relapse. This is not considered a primary treatment. Fee is $50 per session.

*Insurance accepted is limited to Sanford and Blue Cross Blue Shield of any state and is filed through BCBS North Dakota. Insurance is not accepted for DUI or Legal related issues but will be utilized for Individual and Group Sessions. Billing BCBS does not guarantee full coverage of services and client may be responsible for the remainder of the bill, based on their individual insurance plan.
